Comments:

Due to national backorder, we are restricting IV phosphorus (sodium phosphate and potassium phosphate) to patients with phosphorus levels <2.   We will also cease adding phosphorus to TPN bags and all replacement will need to be a separate order written for a rider.   The following alternatives are available for patients who are able to take supplements by mouth or per tube:
