  • Patients >65 years of age
  • Patient < 50kg
  • Renal impairment (CrCl 30-50mL/min)

Conversion

30-60mg IV/IM q6-12 hrs

15mg IV/IM at same frequency (max q6h)

30-60mg IV/IM q6-12 hrs PRN

15mg IV/IM at same frequency (max q6h)

10-20mg PO q4-6 hrs

10mg PO at same frequency (max q6h)

10-20mg PO q4-6 hrs PRN

10mg PO at same frequency (max q6h)
