Restrictions for Oral Ketamine for Intractable Pain control in Palliative Care patients.

  1. Orders for oral ketamine must be prescribed by a Palliative Care Physician.
  2. Patients must have a DNR order
  3. Patients must be on 5-West, PCU units at OLOLRMC
  4. Only for patients 18 years of age or older
  5. All Ketamine orders should include clear goals for pain scores as well as additional parameters for contacting providers if necessary
